In Utah Valley where more babies are born than any comparable area in the United States (39 per 1000 population compared with 15 per 1000 nationwide), the term infertility may have an incongruous ring and associated problems may seem insignificant. Not so, according to the American College of Obstetricians and Gynecologists and specialists at Utah Valley Hospital in Provo. Robert P. Romney, M.D., one of those specialists, says that probably 15 percent of all married couples are infertile, for at least a long period of time, and that virtually every one of them want to have children as much as the couples who have a dozen. "Not only are these individuals deprived of the pleasure and gratification children bring." said Romney, "but there are usually some very serious even more so in this area because we side affects want children mavbe more than anywhere else." Infertility, he says, can lead to severe stress and tension in a marriage. This is particularly true of a couple who has learned that they have little or no chance of conceiving a pregnancy. Many have feelings of sexual inadequacy, or failure, or guilt. They tend to blame themselves or each other, causing great emotional strife at the very time when a couple should be especially supportive of each other. family and friends often compound the problem by asking such questions as "When are you going to start your family?" or "Oh, you mean you don't have iny children yet?" Though such remarks abmay be made in jest or in innocence, they are solutely devastating to the childless couple. "Males especially," said Dr. Romney, "feel their virility is in question if they don't have children. Siring children should not be a measure of masculinity, but it often is perceived that way. The husband almost always sends his wife in first to see why they are not having children even though on one in four problems involve the male. Quite often it's a combination of things, and that's how we approach it. With patience and a careful, systematic approach, many of the problems can be corrected." Three couples Dr. Romney spoke of who have successfully conceived after years of childless marriage live here in Utah County. One couple, John and Lin rt Well-meani- for eight years before their little girl, Janey, came along. Marge and Jeff were married for ten years before Cindy was born. The third couple, John and Diane, discovered that surgery was needed before conception could occur. In each case, several years of careful, patient effort was required to conceive a child. All three couples credit Dr. Romney with something just short of a miracle. Dr. Romney insists, however, that there is nothing miraculous about the help he and other infertility specialists are able to give. "We have had a very high level of success," he said, "and it is due to the careful and organized approach to the individual couple's problems and a great deal of patience and cooperation on the part of the prospective parents." This process begins with a letter to the couple, indetailing procedures they can expect to follow and A to how questionabout structions begin. lengthy naire asks dozens of intimate questions about sexmakuality, contraceptives, menstrual periods, love ing, family history, wife and husband's medical history, how the couple feel about having a baby, medications, work environment and personal habits. Careful instructions, are also given regarding specimens that the prospective parents must collect. All of this is done prior to the first visit to the doctor's office. By doing all this ahead of time, explains Romney, the physician knows a great deal about the couple before he ever sees them. Tests and treatment can begin on the first visit, with little or no time lost on paperwork and preliminary data that done might require as many as four visits if it were one step at a time in the doctor's office. Once tests begin, all causes of infertility and all factors necessary to conceive are considered.
