All children 5 and over who are listed as dependents on the 1987 federal tax return must have a Social Security number. To apply you must show a state certified birth or baptismal certificate, and one form of recent ID for each child. This can be a shot record, medical records, prescriptions, school or church records, last year's tax return, etc. Children under 18 need not be present. This service is free.
