Show HILL TOP TIMES November 6, 1981 M Do ft All military ft D3 members and their families should start obtaining their medical history forms from the Family Practice Clinic at USAF Hospital, Hill in order that they will be ready to be enrolled on Nov. 15 and have their own family doctor assigned. Starting that date, each service member and family will beassigned a physician to call their own "family doctor." That physician will be able to treat the entire family for most illnesses and, when necessary, refer a specialist and still follow treatment and progress. This physician will become gSlDOT) familiar with every member of the family and at the same time, the family can get to know him. For those who may be thinking "what is a family practice physician and what is his training," the answer is simple. The family practice physician is a special kind of doctor. He is a specialist trained in five broad areas of medicine: pediatrics, internal medicine, surgery, obstetrics and gynecology and psychiatry. This broad training gives him the "know how" to handle the majority of a family's health problems. d! (QHnSHolw Win) A key element in family practice is Page 11 eft IT medical history form available at the Family Practice Clinic and following guidelines for proper use of the clinic. A clinic guide will be provided when the service member and family enrolls "continuity of care." A family physician is specially trained to provide this continuity. He is the focus for the entire family's health care and is YOUR doctor. Even when prescribing consultation with another specialist, responsibility for supervising you or your family's health care is never relinquished. Just as the family physician has a responsibility to you, you also have a responsibility to the program. The service - member must come to the hospital to enroll the family. This can be accomplished by completing the 'in the program. Information about the Family Practice Clinic and the name of each physician and physician family assistants who will be working with him will be available in individual orderly rooms. For more information about the Family Practice Clinic, Call Ext. 75503 or 75504. There will be no change in premiums already withheld. According to officials in the Entitlements and Benefits Function, Civilian Personnel Division, this extra benefit doubles the amount of life insurance payable if a civilian employee is age 35 or younger. However, beginning on the. 36th birthday, this extra benefit decreases 10 percent each year until at age 45, there is no extra benefit. Following is how the insurance increases at different ages: New bl ue jacket change receives better guidance Recent policy changes on wear of the lightweight blue jacket has brought numerous queries to AF Headquarters, Wash., D.C. The guidance was intended to better define wearing the uniform in public, especially to official events where protocol and good judgement require adherence to high standards of dress. ,.. The following comments expands upon the basic rationale for changes in policy and should answer any questions: Wearing the uniform while traveling by commercial means: Combination one is to be worn by all Air Force personnel who decide to wear a uniform while traveling by commercial means in conjunction with PCS, TDY, or leave. The combination one is not required when commuting to and from Apwork via commercial means. propriate major command installation commanders may establish exceptions where wearing the combination one would be inappropriate because of climatic conditions or other unique situations.
