D BROCKBANK Rites Ries Being Planned I Funeral arra arrangements for Dr I Heber David Brockbank 49 1733 I Harvard avenue well known known wel-known Salt Sal Lake dentist and Utah State Dental Dental Den Den- tal association official who died Monday night were being made Tuesday Dr Brockbank died in a Salt Sal Lake hospital of septicemia after I an illness iness of several weeks A leader in dental association affairs Dr Brockbank had served as secretary vice president and president of the Salt Sal Lake Lae District I Dental society was a member of the state board of dental examiners examiners examiners ners was president and secretary of the Utah State Dental Golf Gol association was a former editor of the Utah Dental Magazine and at the time of his death was chairman chairman chair chair- man ma of the Dental Service soc society ety in Salt Sal Lake City Dr Brockbank was born in Spanish Fork December 27 1891 1801 a son of Joshua and Sarah Ann Jex Brockbank He attended the Spanish Fork schools Brigham Young university the University of Utah and was graduated from the Chicago College of Dental Surgery in 1917 He was a veteran veteran veteran vet vet- eran of the World war and held helda a commission of captain Active in L D S church affairs he was a member of the tabernacle tabernacle tabernacle taber taber- nacle choir for several years and led the Logan Square branch L D S choir while studying in Chicago Surviving are his widow Mrs Olivia Olvia Swenson Brockbank a son David Nevin Brockbank Salt Sal Lake City two sisters Mrs Eliza Elza Hales and Mrs Lillian Lilan Boyack Spanish Fork and his brothers W.
