Employe's full name, address, date of birth, if under 19 years of age, hours worked each day and each week, basis of pay, and the regular rate. 2. The wages paid for each work week at the regular rate of pay. 3. Additional wages for each week attributable to the payment of overtime in excess of the regular regu-lar rate. 4. Additions to cash wages at cost or deductions from stipulated wages in the amount deducted at the cost of the item for which de-ducion de-ducion is made, whichever is less. |
