These fundi are made available through the Social Service Block Grant Act, hereinafter referred to as the SSBG Act. The people of each community, through their 16 I Key Economic Indicators for San Juan County locally elected officials, determine: A. What needs or problems they are faced with that fall within the scope of social services B. What their priorities are in trying to address these needs or problems C. What programs might be most eflerfive in meeting those needs FEDERAL OOA1-- S AND OBJECTIVES SSBG funds are directed to helping families and individuals achieve one or more of the goals: nomically or remain B. To protect children and adults who cannot protect themselves from abuse, neglect, ex- ploitation; C. lb preserve, rehabilitate, and reunite families (help families stay together) D. lb prevent and reduce inappropriate institutional care as much as possible by making home and community service available; E. lb arrange for appropriate placement and services in an institution when this is in an individuals bast interest. SERVICES WHICH CAN BE PROVIDED WITH SSBG FUNDS SSBG fundi can be used to provide social ser--i ices to low and moderate income individuals and families. The following services may be provided by SSBG funda: Congregate Meals Counseling and Outpatient Day Treatment Domestic Violence Employment, Education, and Training D.D.S. Agency, address, telephone number, and contact person. B. Federal goals the request is meeting. C. A program description which will include. 1. Problem! to be addressed 2. Population to be served. 3 Magnitude of the problem. 4. Numbers to be served. 5. Services to be provided. 6. Program goals and objectives. 7. CONTRACTING PROCEDURES 17ie San Juan County Commission eels the priorities on allocation of funds based on the recommendations of the SSBG Human Services Board. Thia board consists of individual appointed by the Commission and represents all areas in San Juan County. The SSBG Human Services Board will meet June 22, 2000 at 1:00 in the San Juan County Library in Blanding to review the applications. Applicants may have a representative attend the meeting to explain the request or answer questions. It is not necessary to be in attendance to have your application considered. After hearing the proposed applications, the SSBG Human Service Board will make recommendations to the San Juan County Commission who, in turn will mske the final decision as to the funding.
