Provo, Utah, August 4, 1922. that the following facts are true On about Nov. 12, 1921, while residing in Kansas City, Mo., I suffered an attck of amnesia (loss of memory) and in this condition I roamed over the country using the name of Thomas A. Anderson. On. Feb. 2, 1922, 1 was registered under the name of Anderson at the Auditorium Hotel, Denver, Colo. I was approached on the street by a man who claimed he knew me as Hay-waThompson. He stated that I had served with him in the active forces of the U. S. Marine Corps during the World War, and that I had received a discharge for disability due to injuries incurred while in the service. This meeting caused me to realize my condition and I sought relief. It may be well to mention that during this period I retained the sense of reason a fact that made the affliction a terrible one. I suffered an attack of total blindness on or about Feb. 16, 1922. I was taken to St. Luke's hospital, Denver, and called the best surgeons available, also the leading nerve specialists. Four days later my sight returned of its own accord. During this time I had no memory of events prior to Feb. 2, 1922. The specialists advised that my only avenue of relief would be through an operation on the brain as I was suffering scar adhesions of the dura mater caused by a compound depressed fracture of the skull. I realized the necessity of having this operation performed by a surgeon who was thorough. I went to Boston, Mass.; New York, Philadelphia, St. Louis, Kansas City, - Rochester,-and-oth- er numerous to mention. I consulted the leading points brain specialists of the U. S. All advised an operation but would not consent to perform it. As is the usual attitude, as a last resort, I concluded to chance Chiropractic. Dr. Leo L.
