Patients need only to fill out an medication request form (Air Force Logistics Command Form 7430) located at the pharmacy entrance and submit it to the pharmacy window to receive their medication. Donations may be brought to the Hill AFB Chapel, Bldg. 475, weekdays from 7:30-- 4 p.m. The hospital serves part of the following states in addition to Utah: Nevada, Idaho, Montana, Wyoming and Colorado. Outpatients totaled 140,259 last year along with 8,064 admissions. The Utah Department of Commerce is issuing this warning after receiving reports of people attempting to raise money to benefit the troops in various ways, who are not legally registered to solicit funds for these purposes.
