Amended tax returns filed by Oct. 17 will also qualify for rebates. It is to the advantage of taxpayers to pay any outstanding state income taxes by Oct. 17 in order to increase the amount of their rebate. Any amended return received after Oct. 17 will not be considered for rebate purposes. All rebates will go through the state's GOTCHA program. This program in the Division of Finance compares all state refunds against such things as delinquent child support payments, outstanding student loans, unpaid loans from the failed thrifts, unpaid taxes, etc. Any outstanding debts in these areas will be deducted from an individual's rebate. The first of approximately 500,000 income tax rebates will be mailed by the Division of Finance the first part of SEptember. It is anticipated that all rebates will be mailed by the end of The Utah State Tax November f Commission will compute the rebate amount for each taxpayer. Utahns do not need to apply for the rebate. The rebate check will be mailed to the address used on the 1987 income tax return. If a taxpayer has moved, he or she should call the Utah State Tax Commission with the new address. The number is Taxpayers will also need to give their social security number. The rebates were approved at the special session of the legislature in response to Governor Bangerter's request to return to Utah taxpayers in $82 million approximately projected state income tax surplus. It will cost the state $286,000 to process and mail the rebate checks. The rebate is 12.5 percent of state income taxes paid. If the taxes paid in 1987 were less than $80, the rebate would be under $10 and no check will be issued. Instead the taxpayer will be able to take a credit on 1988 income taxes in 1989 for the amount of the rebate. The rebate will only be calculated on taxes paid. There will be no rebate on taxes owed but not paid mm Helen Ellison 530-697- . gon, Ellison attended high school in American Fork. She married Harold B. Ellison and settled down to raise a family that eventually included one daughter and six sons. After a number of years passed, she returned to school at Utah Technical College in Provo to earn her nursing degree. Ellison keeps busy in a number of ways in addition to her nursing Ellison says she loves being a nurseespecially the contact she has with people. "From the time I was a young girl, I loved caring for anything that breathed-str- ay cats, homeless dogs and kids with I dreamed of skinned knees-a- nd being a nurse," she explains. An actual career in nursing didn't begin for Ellison until midlife when she earned her LPN degree. She joined the American Fork Hospital nursing team in 1979 and served in the nursery, women's center, medical floor and surgical floor before joining the Same Day Surgery and endoscopy lab staff. Although she was born in Ore- - career. She has served on several hospital committees including the Policy and Procedures Committee and the Hospital Week Committee. Carroll Carroll, 80, of Midway, died Monday, Aug. 8, 1988 in Pleasant Grove. She was born Sept. 30, 1907 in Lindon to Isaac and Hattie May Buzzant She married Harris. Ray LaMont Carroll Oct. 31, 1928 in the Salt Lake LDS Temple. He died March 1, 1960. She was a member of the: LDS Church , and served, in the Primary organization. She spent her childhood days in Lindon and attended school there. Following -- m Pauline W. Dotson Dot-so- For almost nine years, nurse Helen Ellison has been serving patients at American Fork Hospital. Her efforts are being recogas she is nized by her honored with the title of August Employee of the Month at American Fork Hospital. The entire Same Day Surgery staff at the hospital, where Ellison now works, nominated her for the Employee of the Month honor. "Helen contributes to the great (atmosphere) in Same Day Surgery," says one fellow nurse. 1 do not know of one complaint where she has offended a patient, staff member of physician. She was a member of the Church of Jesus Christ of Latter-da- y Saints and served in many positions. Some of her many callings included: Presidencies of Relief Society, Primary, MIA and Sunday School, Teacher, Librarian, Visiting Teacher, and she served in the Stake Provo, Janet F.
