Show HEALTH DEPT. WILL ISSUE FREE BIRTH CERTIFICATE Notification of birth registration cards are no longer issued to parents par-ents of new-born babies. Formerly, the Division of Vital Statistics, Utah State Dept. of Health issued free of charge the Notification of Birth Registration, which most parents believed to be a certificate of birth. However, it was merely notice of the fact that the birth had been registered, and it did not have legal value. If, for any reason, parents require re-quire evidence of their baby's birth the State Dept. of Health will issue a certified copy of birth certificate upon receipt of written application, applica-tion, showing name, date of birth, place of birth, father's name, and fee of one dollar. This certified copy is acceptable to the Federal Government, the armed forces, and any agency which requires evidence evi-dence of birth. |
