As long as any part of the 21 days period remains, members requiring further hospital care are entitled to an additional 40 days at 50 of full Plan benefits. Full benefits, including 21 days of hospital care, renew 90 days after last hospital discharge. SERVICE BENEFIT FEATURE Participating physicans will accept Blue Shield for the surgical, medical and fees as payment-in-fupsychiatric services outlined above if your family s annual income does not exceed $7,200.00 ($5,500.00 ll for single persons.) If your annual income exceeds these amounts, your participating physician may charge the difference between his normal fee and that provided by Blue Shield. More than 98 per cent of all Utah medical doctors participate in the Blue Shield plan. Participating hospitals of these Plans will provide you the service benefits of the Blue Cross Plan serving the area in which you are hospitalized. If you are admitted to any other licensed general hospital outside the State of Utah, you will be entitled to receive the same full benefits as are provided in Utah participating hospitals Cash benefits promptly paid to any hospital in Utah. Licensed doctors of medicine who practice outside of the State of Utah receive the same fee as is paid to a participating physician for similar services. Payments will be made to any licensed doctor of medicine in Utah for surgical or medical cases requiring emergency care.
