Whereas seveial years ago rheumatic fever was often never diagnosed and many crippled hearts resulted, nowaday there is a problem of in many cases, and children being needlessly put to bed for several weeks wuh their activity being curtailed The public commonly makes a diagnosis, or assumes rheumatic fever to he pnsent, because of questionable fcveis and vague leg aches. 'I he enor is compounded when a blood sedimentation test is found to be ele-ated 'I hew s mptoms alone are not sulluient to diagnose rheumatic fever being present. Admittedly, there are a certain mini he i of cases that develop heart disease without Wing recognized, but usually this means lati of medical caie during and following a streptococcal infeition. There is no speuUc test is e v itudied ure is the prevention of streptococcal disease. This is achieved through the daily administration of oral penicillin or sulfadiazine in small doses. Another type of slowly absorbed penicillin is given in monthly injections. Any patient with definite rheumatic fever should continue this preventative therapy for a number of yean. Contrary to some beliefs, the type of streptococcal germ that causes rheumatic fever rarely becomes penicillin resistant. Prevention of subsequent attacks of rheumatic fever is the most important thing that can be done to prevent heart disease. Prevention of the initial attack of rheumatic fever is accomplished by at least ten days of penicillin therapy for any streptococcal infection. This is felt to be due to the newer knowledge and prevention measure that we now have available. With recovery from the initial attack of rheumatic fever, the patient has an 80 percent chance that he will recover with no detectable heart damage or such a slight degree of damage that h physical activity will not be restricted in any way. It is the patient with repeated at-- t ac ka of rheumatic fever who is left w ith the severely damaged heart. Treatment of rheumatic fever is still being subjected to research and analysis. Red rest is an unchallenged therapy. However, there is now evidence that in the past bed rest has been used for longer periods than are necessary. Salicylate drugs and aspirin are also used under proper medical supervision. The latest treatment is the use of the newer hormones such as Cortisone and the one known as ACTH. The best treatment, or the best combination of treatments, is atili being throughout the woild. More important than the treatment of rheumatic fever, which is only partially successful in tome cases, is the prevention of the disease, which is very successful.
