What should I do? When I the analysis of patient narratives, she adds. Literature places ethical issues in a more complex human dynamic. Instead of being stripped to the abstract, patient narratives come with all the messy complexities of life, she says. Jones has found that even books that dont focus directly on medicine like Norman Macleans ,A River Runs Through It, can bring ethical issues into relief. At the core of that work is the problem of how to help somebody who doesnt want to be helped, which is a frustrating and common phenomenon doctors face, she says. She also conducts readings on material written by doctors such as the poetry of William Carlos Williams, a great American writer who was also a practicing pediatrician in his hometown of Rutherford, NJ. These works often allow physicians to openly discuss and work through practical challenges and emotional conflicts they encounter from day to day. Certain works can evoke awareness and a better understanding of how to give bad news or cope with mistakes in treatment and judgment errors, she explains. At the heart of Jones enterprise, however, is educating medical faculty, students and practitioners to read in the fullest sense. Reading requires and develops interpretive and analytical skills that increase certain clinical competencies like a doctors diagnostic abilities, she says. Jones sees the patients story as the center of medicine. When a patient walks into a doctors office, he begins by telling a story about his physical well being, medical history and experience with pain or illness, she explains. A physician must interpret this narrative to discover the diagnosis and best course for treatment and then be ql?le. to relate back to thepatientra narrative in clinical terms, The healing process is very much one of stories, the patients story must become held as the physicians, she says. Jones stresses in her workshops the importance of narrative as a non numbers way of knowing. Medicine prides itself oh being a science, but questions linger with which it must contend, as must art, she says. Science and narrative are two different methodologies for getting at the truth of what it means to be human. And its better to be able to use both, she believes. While in Salt Lake, in addition to her reading groups, Jones has produced a readers theater version of a play she wrote about her experience as the primary caregiver for her late grandmother Vho suffered from Alzheimers. If you have on hand more than you need, there are several things you can do. Read the storage and disposal section of the label for special disposal instructions. Use the pesticide according to label directions to dispose of the pesticide. This practice is not always feasible, especially when die label limits the frequency of the application. If you cannot use it up yourself, then consider giving the pesticide, in its original container, to another applicator to use according to label directions. Most county and city health departments accept home and garden pesticide products for disposal. See if this option is available in your area. Pesticides for disposal must be safely stored until an acceptable method for disposal is available. This may involve storage for extended times and safe storage is difficult. When there are requirements for the proper cleaning of pesticide containers before disposed, directions are in the storage and disposal section of tiie label. Most commonly cleaning is accomplished by triple rinsing containers with water or shaking out paper or plastic bags. To triple rinse: Empty pesticide container into spray tank and allow container to drain for 30 seconds. Add rinse water to container full. Rinse conso it is tainer thoroughly, pour rinse water into spray tank and drain for 30 seconds. Do this procedure three times. DO NOT pour pesticides down the drain or into water. Reading these works gives one an understanding of the way cultural attitudes and ideas about illness have changed, Jones says. For example, cancer and mental illness used to be taboo topics of conversation whereas a heart attack was thought of as a for successful, business people, she type-says. Now, due in part to patients going public with their stories, the taboo on diseases such as breast cancer and schizophrenia has been lifted. By exposing these cultural biases, literature forces physicians to take a critical look at their own prejudices and evaluate the quality of care afforded different patients, says Jones. This critical and greater capacity for empathy, in turn, shed light on the strengths and inadequacies of the entire health care system. Through reading narratives of patients who have suffered from chronic mental or physical illnesses, the need for better longterm care, which the managed care system doesnt support, becomes painfully clear, Jones says. Managed care can do a lot toward cutting costs, but these narratives demonstrate that the human price is often too high, she says.
