Mr. and Mrs. Zen Boothe, Mr. and Mrs. Lee Boothe and Mr. and Mrs. Carl Pace attended a Boothe Christmas party Friday evening in Salt Lake at the home of Mr. and Mrs. Duane Lawrence (Carolyn Boothe) a delicious turkey dinner with all the trimmings was served to 40 guests, members mem-bers of the Cal Boothe family, the evening was spent visiting and exchanging Christmas gifts. Mr. and Mrs. Zen Boothe attended the ceremony and reception re-ception of Bob Thomas and wife Paulette, Saturday evening in American Fork. "Congratulations "Congratu-lations to Bob Thomas and Bride". Medicare can pay medical insurance in-surance claims only within a certain time after treatment or other service. If you have Medicare medical insurance and received covered services during the period Oct. 1, 1968 through Sept. 30, 1969, claims must be filed before the end of 1970. After Dec. 31, 1970, Medicare cannot pay these bills. A completed "Request for Medicare Payment" form should be sent to Medicare at 2455 Parley's Way, Box 270 Salt Lake City, Utah 84110. If the doctor or supplier does not complete his portion of the form (Part n), an itemized bill must accompany the "Request for Medicare Payment". Either the itemized bill or Part II of the claim form must show the date, place, and description de-scription of each service and the charge for each service. Anyone needing claim forms for Medicare medical insurance in-surance or related information should contact the social security representative who visits your area or " call the Provo Social Security Office.
