Lasers are used to j fine wrinkles (facial resurfacing), lessen acne scarring, remove vascular (red) lesions, pigmented lesions, hair and i tattoos. S Facial treat i , : DR. SMITH: , j . : . : , ! resurfacing There are two types of lasers currently used for facial resurfacing: The Car- bon Dioxide (C02) laser and the Erbium laser. Slight differences between the two types may cause the surgeon to choose one over the other. Both lasers can remove wrinkles, however, the C02 laser may treat deep facial wrinkles more ef-fectively. The Erbium laser is beneficial in other areas such as the neck and hands, and it may perform better in patients of darker skin coloration. Both lasers provide excellent results softening wrinkling, lessening acne skin. scarring and treating Laser resurfacing may provide improve- ment over chemical peeling for some people. Laser procedures may be used in conjunction with other facial rejuvenat-- . ing procedures such as face lift or ble- pharoplasty (eyelid tuck). Facial resurfacing with lasers allows in - : -- 'i ; i j sum-damag- ed for treatment of wrinkles and some acne scarring. The laser removes superficial portions of the skin. As the treated area heals, a tighter, more youthful skin develops that shows decreased wrinkles and improvement in skin with superficial acne scarring. People generally heal within 10 to 14 days with the C02 laser while the Erbium laser requires seven days of healing time. Within this time, most patients are able to wear makeup and resume activities. Results of laser treatments are thought to be long lasting and generally will not need to be repeated for many years. The exception is acne scarring. This condition may require multiple treatments, either with the laser or with chemical peels, to provide best results. Post procedure redness, which appears similar to a mild sunburn, may persist for two or three months with the C02 laser, or one to two months with the Erbium laser. tation changes. Pigmented lesions and tattoos Vascular lesions Skin tightening occurs due to the removal of superficial skin layers by the laser. As skin heals, collagen within the skin is reorganized. This reorganization restores elasticity to a more youthful structure. Skin tightening continues up to one year as rejuvenation occurs. Vascular lesions (collections of blood vessels present in or grouped beneath the skin) are conditions present at birth or soon after. Telangiectasias (dilated small blood vessels) is usually visible in the face. In treatment, laser energy is absorbed by hemoglobin in red blood cells. This causes blood vessels to shrink and possibly disappear. Lasers allow treatment of these conditions with little or no scarring, which has been the case with more traditional surgical removal and repair. Multiple treatments may be necessary to remove lesions. bruising may occur, but generally resolves without specific treatment. Newer lasers produce less bruising and pigmen Post-treatme- nt Pigmented lesions are areas that have increased melanin (pigment that gives color). People with a darker complexion have more melanin within their skin. Pigmented brown lesions contain more melanin than surrounding skin. Lasers target melanin and allow the body to remove the pigment. Some swelling and perhaps minimal scabbing may be present for one week. The Nd:Yag laser works well for removing these lesions. The Nd:Yag laser also works on colors in decorative tattoos in a similar fashion to the pigmented lesions. Tattoos generally require four to eight treatments. Tattoos with extensive green coloration are the most difficult to remove. As the laser targets pigments, either in skin or the tattoo, the bodys white blood cells remove pigment debris. Hair removal Technology for using lasers for hair removal is changing rapidly. Perhaps some of the newer lasers may provide permanent hair removal. In most stud ies, permanent is defined as no regrowth for at least one year. The procedure usually requires approximately three laser treatments to provide permanent results as the laser treats actively growing hair. If a patient desires reduction of hair, and not removal, fewer treatments may be required. Generally, patients with darker hair on lighter skin are the most ideal candidates for this treatment. Laser treatments are generally performed in a physicians office with minimal or no anesthesia necessary. Most treatments are not covered by insurance, but one should check with the physician or insurance carrier. People who have lived with scarring, wrinkles, lesions, excess hair or unwanted tattoos for many adult years are pleased to know that laser surgery can improve their appearance.
