A. Cook, of B5I Twentieth street, received a telegram this morning announcing the sad death of Mrs. Helen Chase, a former resident of Ogden. Death resulted from the shock of an amputation of the foot, which became neceaeary because of an accident, sustained some time ago by the deceased. Mrs. Chase died last Monday, according to the Information received here. Mhe was for a number of years a resident of Ogden, where she lived with her husband on Twenty-thir- d street.
